Transcriber's Notes
Street of residence (items 2d and 16b) reads 'Elston' (item 2d could be read 'Eleton'); uncertain—possibly another street name such as Eleanor.
Hospital name in item 1(e) is a very faint stamp; 'PHILADELPHIA GENERAL HOSPITAL' was confirmed on a contrast-enhanced crop (2026-09-27), consistent with the physician's address 'Phila. General Hosp'.
Funeral director's signature reads 'H B Mulligan' (clear on re-check); the address '1119 W Lehigh Av' (written 'Lehh'-like) remains uncertain.
Physician's signature is hard to read: 'Uhle / W H Garrey [?]'.
Registrar's signature and date-received stamp are too faint to read.
Mother's birthplace written 'Germy' (abbreviation as written).
Pencil numbers '255' and '50' written near the certificate title.
