Transcriber's Notes

Mother's maiden name read as 'Barbara Wessell [?]'; the surname could also be 'Wassell' or 'Wessel'.

Date of birth read as 'Aug. 8, 1857'; the day could be 6 or 8 (age 82 y 5 m 25 d at 2 Feb 1940 fits Aug. 8).

Physician's signature read as 'O. W. Aichele'.

Filed stamp and registrar's signature are very faint; the date looks like FEB 5 1940.

Code numbers beside causes of death ('131 L', '93 L') are later annotations.

Informant's signature 'Frederick Knoll' is a scrawl but reasonably clear.
