[Printed vertically in left margin:] WRITE PLAINLY WITH UNFADING INK – THIS IS A PERMANENT RECORD ... [instructions on stating age and cause of death, largely illegible]
Transcriber's Notes
The photocopy is very faint; readings were made from contrast-enhanced crops.
Registered No. read as 11723; this matches the funeral record's permit no. '11-723'.
Informant's address read as '317 [?] W. Spencer St.' (there may be an extra stroke, i.e. '3117').
Undertaker's name is faint: 'Jno. F. Fluehr & Sons, 149 W. Susq. Ave'.
Physician's signature read as 'Pedro J. Carreras' (funeral record gives 'Dr. P. J. Carreras'); his address is faint: '106 W. Susquehanna Ave. [?]'.
Filed stamp date is mostly illegible except the year 1939. Registrar signature partly illegible ('Joseph A. ...').
The faint handwritten '540' [?] near the header and '131', '93C' [?] beside the causes of death appear to be later code numbers.
Cause of death first line read as 'Chronic Nephritis' (faint), consistent with the funeral record.
