W.D., A.G.O. FORM NO. 100 15 July [torn]

Transcriber's Notes

The typed Military Specialties rows are mis-registered: the ruled lines run through rows 2 and 3, so they look struck through, but they seem to be ordinary entries. The years figure in row 3 is mostly hidden by the ruled line. The first digit might be read '2'.

Army code numbers: row 2 '345' and row 3 '050' are typed very close together in the same cell.

A small handwritten mark (an arrow or check) sits in the left margin beside the first two Military Specialties rows.

The Art Loom employer address line 'Phila. Pa.' runs across the bottom rule of the box.

The officer's printed name is broken by a tear: 'VON SCHLIC[..]TEN'. The signature suggests 'Von Schlichten'.

'Seperation' is spelled that way in the typed entry.

The form date at the bottom is cut off after '15 July' by the torn edge.

In the Grade box, something is typed above the '5' in 'T/5' (possibly an overstrike).
